- Your insurance card(s) and photo identification
- A complete list of medications (names & dosages)
- Prior medical records and consultations related to your condition
- If recently hospitalized, a copy of the discharge report
Length of Your Visit
Please allow 1–2 hours for your appointment. Our Physician will spend the time needed for an appropriate assessment and plan. We appreciate your patience—every patient receives the time they need.
