What to Bring to your in-person appointment
- Your insurance card(s) and Photo Identification.
- A complete list of the names and dosage of all the medications that you take.
- Prior medical records and consultations from other physicians who have investigated your medical problem.
- If you have been hospitalized for your current problem, please bring a copy of the discharge report.
Length of Your Visit
Please allow 1-2 hours for your appointment. However, no two patient visits are exactly the same and Dr. Yau will spend time to ensure appropriate assessment and understanding of your care. Please prepare for possible delays with the assurance that you, too will have the time you need with the doctor.
What to expect with your telephone/ video appointment visits
All you have to do is have your telephone with you when it’s time for the call. Please expect to receive a call 20 min before and after your appointment time.
The doctor will attempt to call you three times before you will need to reschedule your appointment. If you have call screening on, your phone device, you may need to disable the feature in order to receive a call from our office. Please do not hesitate to ask our staff for help.
You can expect the same high-quality care you get at our office, and every encounter is recorded in your electronic medical record.
